Motion authorizing the City Manager or his designee to execute a contract with Crest Benefits Consulting, Division of JDW Insurance of EI Paso, Texas for consulting services related to the City's health plans and wellness initiatives to include the possible implementation of a clinic and pharmacy with an initial term of one year for an annual fee of $48,000 and with two one-year extensions for a maximum increase of 5% per year, upon contract execution by consultant.
