developer of S.P.I.C.E. Plaza, Block 3, Lots 1 and 5, for the installation of 926 linear
feet and over sizing of 894 linear feet of 12-inch PVC water grid main lines.

APPLICATION FOR WATERLINE REIMBURSEMENT
I, Rory Chen, owner and developer of pr,posed S.P.I.C.E. Plaza. Block 3, Lots 1 & 5.
Subdivisior, hereby request reimbursement of $31,529.75 for the installation of the water
(distribution main) arterial transmission or grid main in conjunction with S.P.I.C.E. Plaza, Block
3, Lots 1 & 5 Subdivision, as provided for by City Ordinance No. 17092. Said $31,529.75 is the
construction cost, including 7.5% Engineering, in excess of the lot/acreage fee, as shown by the
cost supporting documents attached herewith.
