documents necessary to accept an amendment to a grant contract for
the Health Promotions and Chronic Disease Prevention/Texas Healthy
Communities(HPCDP/TXHC)grant in the amount of $50,000.00, increasing
the total amount to $100,000.00, from the Texas Department of State
Health Services in the No. 1066 Health Grants Fund to provide
funds to implement systems and strategies that address one or
more Texas Healthy Communities priority indicators for the
amended contract period October 1, 2015, through September 30,
2017; appropriating the increased grant amount; and ratifying
acceptance of the amended grant agreement to begin October 1, 2016.
Be it ordained by the City Council of the City of Corpus Christi,Texas:
Section 1: The City Manager or designee is authorized to execute all documents
necessary to accept an amended grant amount of$100,000.00, from the Texas
Department of State Health Services in the Health Grants Fund No. 1066, for the
amended contract period of October 1, 2015, through September 30, 2017, to
provide funds to implement systems and strategies that address one or more
Texas Healthy Communities priority indicators.
Section 2: That the increased grant amount of$50,000.00, is appropriated in the
No. 1066 Health Grants Fund to continue the HPCDP/TXHC grant in the Health
Department.
Section 3: The amendment includes revisions to Section 7 Statement of Work and
Section VIII Special Provisions.
Section 4: Further the City Council ratifies acceptance of the grant to begin as
of October 1, 2016. A copy of the executed grants shall be filed in the office of the
City Secretary.
