· In the odginal contract, Section 2. SCOPE OF WORK shall be modified by t~e addition of the
following scope: design and survey to incorporate Hewitt Drive drainage improvements work into
the Phase I Part A project (as described in the attached Exhibit
· Summary of Fees is amended as described in the attached Exhibit "A."
All other ten'ns and conditions of the Contract and Amendments between the City and Consultant
will remain in full force and effect.

Contract Amendment No. 3 - Revisions to Scope, Schedule and Fee for
Hewi~t Drive Drainage Improvements
Dear Joe:
The following is our request for an amendment to our Contract for Professional Services regarding the scope,
schedule and fee for the work.
Scope: The original scope of the project included preparation of an engineering letter report for the Hewitt
Subdivision area drainage deficiencies (design work was not included in the scope). This report identified drainage
ouffall deficiencies on Hewitt Drive. Our reconu'nendation was to provide a swale along an existing utility easement
between Lots 14 and 15. Storm rmmff would drain to an existing post inlet that would be tied into the new 9' x 9'
box culvert. Design and survey are required to incorporate the drainage improvements work into the Phase 1 Part A
project. The additional scope includes providing the following:
· a topographic survey of the area,
· TCE documentation,
· hydrolog/c/hydraulic analysis to size the swale,
· design,
· cost estimate and
· drawings. The drawings would be incorporated into the Part A plan set.
Schedule: The attached schedule reflects the current and revised design, bid and construction phases for Phase 1, Part
A. This schedule indicates a construction start date for Phase 1, Part B near the timeframe for the completion of
Phase 1, Part A construction.
Fete: The lump sum fee for these previously performed services is $16,345.00 We look forward to receiving this
contract amendment.
