SCOPE OF SERVICES, Part B. Additional Services shall be modified by the addition of the following scope: O. N. Stevens Water Treatment Plant, Phase 2, Filter Drain, Yard Pipino, Wash Water Filter to Waste RNW-FW) (Prolect No. 8375) - Consultant will provide additional Resident Project Engineer Services. Laughlin-Thyssen's original contract time was to end in December 2004. After lengthy negotiations on various change order requests, a new contract completion was se/at April 1,2005. LNV Engineering and Construction Services have been recently notified that the actual completion ol~ [he project would be June 2005 at /he earliest, and August 2005 at the latest. This extension of construction completion requires LNV Engineering to provide an additional six (6) to eight (8) months of Resident Enclineerinci and Inspection Time. LNV Engineering estimates that rot both positions, an average of twenty (20) hours per week, respectively, will be required. O. N. Stevens Water Treatment Plant, Phase 3, Elevated Tank (Protect No. 8376) - Amendment No. 11 authorized additional inspection services for a Certified Welder and Tank Specialist for inspection between the original scheduled completion date of January 15, 2004 to August 6, 2004. Due to the Contractor's difficulties painting, replacement o[ three (3) wind rods, repair o[ the 42-inch inlet / outlet piping, and concreLe repair, construction occurred intermittently until March 2005. It is estimated that this project will be accepted in April 2005. LNV Engineering requires additional Inspection Services for the Tank Specialists' inspections, Certified Weld Inspections, and the non- destructive testing that was not compensated for in the original contract and amendment since August 2004. Stevens WTP: Stevens Plant Wash Water System Phase 2 (Project No. 8375) Stevens Plant Wash Water System Phase 3, Elevated Tank (Project No. 8376) Amendment No. 12 Dear Mr. Escobar: Please allow this letter to serve as our formal request to amend our Contract for providing additional services under this contract. For Project 8375, as you are aware, Laughlin-Thyssen's original contract time was to end in December 2004~ After lengthy negotiations on various change order requests, a new contract completion was set at April I, 2005. LNV Engineering and Construction Services have been recently notified that the actual completion of the project would be June 2005 at the earliest, and August 2005 at the latest, This extension of construction completion will require LNV to provide an additional 6 to 8 months of Resident Engineering and Inspection time. LNV estimates that for both positions, an average of 20 hours per week, respectively, will be required. Therefore, LNV requests additional fees of $108,160.00 to complete these tasks. For Project 8376, Amendment No. 11 granted additional funds for a Certified Welder and Tank Specialist for inspection between the original scheduled completion date of January 15, 2004 to August 6, 2004.
