'"he following City i~aff, rnembem are authorized to Initiate a support call: · Larry Mesaer · D~n Chute · Bill Burnham · Amber Johnson The City will classify eupport requests at ~ t:lrne of th~ contact with EMA staff. The priority level~ are as follow~: · High - GIS application or Interface is unusable (ongoing system or server o~[age). · Medium - Users ere able to work but components of the appllcoflon ere not performing aa expected; Includes performance reliability Issues such as recoverable o,Jtages. · Low - Enhancements, general admlnlstmUo~, and eoflwam patches. EMA will address requests In order of priority. EMA will assign an lesue number and brief desorlpflon to each support request. The deeorlptlon and Issue number will be u~ecl to reference Ifte Issue and to ~eport on the status of the Issue. EMA staff may request additional Information for clarification. EMA will notify Larry IVleeser when work Is completed (Issues dosed) under this support agreement. John Schlebold will be EMA's point of contact for ell GIS support requests. For any requested___ work that ,s anticipated to be $2,000.00 or mom. EMA will get email authorization to proceed from the City MIS D~rec~or Support sen,'icee Include, but are not limited to: · Answering questions from City steff. · Fixing production related problems. · Appyng software patches. EMA shall not be held liable for defec~ or damage to ~ City system caused by Inherent defects wffi~ln the patches provided by ESRI, Inc or other third party software vendom. · Testing changes. · Assisting City staff w~[~ application changes. · Writing or assisting City staff with reports. · Responding to other requests by City staff for support of the GIS end Its Interfaces.
