I~tbl ic Hetdth System ( LPHS } Sen'ice Delivery Plan, dated May 27, 2004.
I'he assessment and/or evaluation activities must include a standaxd that is identified in
PKRPORMING AGENCY'S FY 05 LPHS Service Delivery Plan. Acceptable standards include
the National Public Health Performance Standards approved by thc Centers f~r Diseatm Control
and Prevenlion, Perfu,mance Standards developed by the Texas Association of Local I-Ieall~
Officials, Healthy People 2010, and may federal, state or IGc, al law or regulation ig:lvm~ing file
deli,err of essential public health services. Other evaluation method~ utilizing standards not
listed in this contract Attm:hment must be pre-approved by RECEIVING A~CY.
PERFORMING AGENCY shall c~)mply with all applicable federal and state laws, rule, a,
~cgulaiJons and standards including, but not limited to, th~ following:
· Chapter ~'~- 11 of the Healthy People 2010;
· Section 121.002, Texas Health & Safety Code, definition of m ~a~tial public heallfi
services; and
· Government Code, Section 403.1055, "Permanent Fund for Childaza and Public Health".
