2 SCOPE OF WORK. "CONSUL T ANT" shall provide services for the PROJECT in
accordance with the accompanying Letter. Scope of Services, and Fee attached as "Exhibit A":
Additional DesiQn Phase, Bid Phase, and Construction Phase services associated
with Oso Trunk Main Manhole Replacements Proiect (Proiect No. 7357).
3 FEE. The "CITY" agrees to pay the "CONSULTANT" for services provided in accordance
Nlth Exhibit "A", Scope of Services. and Fee under this AMENDMENT NO. 12, a total fee not to

Oso Trunk Main Manhole Replacements - Project No. 7357
Dear Mr Trejo;
Your recent E-mail requested that HOR Engineering prepare a scope of services and fee
proposal for the replacement of six manholes along the Oso Trunk Main. Four Manholes are to
be in the base bid and the other two as separate additive alternates.
The base bid manholes are located at
· Curtis Clark at Nelson (MH 7882)
· Curtis Clark at Everhart (MH 7876)
· Bonner at Everhart (East) (MH 2281)
· Bonner at Everhart ('Nest) (MH 2280)
The Alternate Bid Manholes are located at
· Curtis Clark at Staples (MH 8139)
· Curtis Clark at Betty Jean (MH 7917)
Since four of these manholes are within the pavement of arterial streets and one is within the
pavement of a collector street, construction traffic control will be a major part of the work effort in
the plan preparation.
Based on similar recent projects of a similar nature, we estimate that the construction cost of the
project to be in the range of $2-2.25 Million.
The City provided videotapes of several manholes to aid in evaluating the conditions. These
tapes did not show specific conditions particularly whether the HOPE liner runs through the
manhole. HOR will, through a subcontractor, conduct additional videotape inspections to
ascertain specific conditions related to the design of the project. This work will be considered as
a Additional Service
HOR is pleased to assist the City with this project. We propose that this work be authorized as
Amendment No 12 to our current contract Our fee proposal for this work totals $82,031.00. The
fee breakdown by phase is

Our proposed approach to the project is
A. DESIGN PHASE
1) Visit each site to locate the manholes in relation to the pavement, travel lanes or other
surface features,
2) Conduct video inspections of the manholes through the use of a subcontractor.
3) Prepare construction docum'ents for bidding including traffic control plans for each site.
4) Prepare opinion of probable cost.
5) Attend two progress meetings
6) Submit 100% package for review.
B. BID PHASE
1) Attend prebid meeting.
2) Address contractor questions,
3) Prepare Addenda
4) Prepare bid tabulation
5) Analyze bids
6) Review contractor qualifications
7) Recommend award
C. CONSTRUCTION PHASE
1) Attend preconstruction conference
2) Recommend testing laboratory
3) Provide clarification to plans and specifications
4) Review submittals and laboratory reports
5) Conduct periodic site visits during construction. (not to exceed six visits).
We propose to have the 100% bid package ready for review within 60 calendar days of
authorization to proceed with the work.
We propose that this work be added to our current contract as part of the Additional Services
section. Should this proposal be acceptable, please process the necessary amendment to our
contract so that we can begin work,
