Contract Amendment No.7 Request
Dear . Angel:
The tollowing is our revised request for an amendment to our Contract for Professional Services regarding
the scope. schedule and fee for the subject project.
Scope.
Request your approval for additIOnal Basic Services associated with Reid Drive Bid Set revisions made to
intersection treatment, ADA compliance route and Additive Alternates 1 and 2, and extended Construction
Administration Service~ for the duration of the project from 40 weeks to 130 weeks. We propose to
provIde these Basic Service~ for a lump "urn of $20,750.
Further request your approval 01 Additional Services for the construction of this project (20 hours/week for
130 weeks) with Mr. PhilIp L. Boehk, P.E. assigned as the City's Project Representative. The additional
fee for this serVIce will be partIally offset by your authorization to use existing construction observation
funds (City ProJect 2174 Contract Exhibit /\ - Part A Additional Services Line Item No.7 remaining
balance 1$8,900) and Part B Additiona, SerVIces Line Item No.9 ($139,500).
A second Additional ServIce will entail the existing tree condition evaluation along the planned
constructIon route of Trunk Mam B-2 ~ ith Cluster 6 Neighborhood Street Improvements and Trunk Main
B3 We intend to engage the services of Gignac Landscape Architecture to assist in our evaluation.
Representatives from GEl and Gignac Landscape Architecture will assist the City in the preparation and
partIcipatIon ill a public neighborhood meeting prior to the commencement of this phase of project
constructIon
~We propose to proVIde these t\.\ 0 AdditHmal Services for a lump sum 5238,540 based on using previously
mentioned available contract dollars. The total Contract Amendment No.7 amount would be $274,590.
