Please accept this letter as~~ur request for an amendment to our current A/E professional services
~ontract for the above referenced project as described herein. The Contractor, R.S. Black Civil
Contractor~ Ine./Machiner~ & Materials [nc. (a joint venture), began const~uction on June 19,
?O(16 The revised contract period runs through July 6, 2007.
Requeated Items to be Amended to the Existing A/E Professional Services Contract:
Exten n of tlie Constructian Observation Services and En~ineering Supnort Per Month
From ,~nu~v 12, 2007 through June 30, 2007
• Daiiy construction observation ~ervices. Time allotted is 15-18 hours per week for Inspector.
•[nterpretation and clarif eation of plans and specifications.
^ ~'~nsult with City during constn~ction
^ Monthly jobsite meetin~ with G~t~y and contractor plus preparation.
^ fnterim jobsite visits anc~ inspection~
^ Prepare change orders.
• T~e cost for this additional serv~ce ~s ~60,810.00 per attached schedule.
