Pilot Program for Assessment /Case Management Service ,~ ~, .~
Memorandum of Understanding '
This Memorandum of Understanding is entered into between City of Corpus Christi acting
through its Pazks and Recreation Juvenile Assessment Cener ("JAC") and Padre
Behavioral Hospital, Corpus Christi Office (Padre).
Purposes:
The purposes of the MOU is to allow wmmunication between JAC participams and JAC
Case Managers while the youths aze at Padre Behavioral Hospital and for JAC to conduct
short-term 3 month case management service.
Fiscal Year of Service:
June 23, 2008 through August 29, 2008
Termination:
• Either party may terminate the MOU on thirty days' written notice to the
other party, at the address provided herein.
Juvenile Assessment Center will perform the following services:
• JAC will meet with pazents to obtain parental consent for youths to participate
in JAC case management, to allow JAC to view school records, to allow JAC
to be able to refer youths for services; and consent for JAC to provide case
notes to Padre.
• JAC will make return referrals to Padre inpatient and outpatient services as
deemed appropriate by JAC.
• JAC will provide case management services to include school visits, office
visits at JAC and home visits, appropriate assessments as appropriate at least
twice a month for each month up to a time period of 3 months.
• JAC will provide Case Management services for all eligible individuals,
limited to 25.
• Announcements of pending trainings and events.
• JAC Services to be provided by persons certified as Case Managers by
American Institutes of Outcomes Case Management.
• Upon written consent of pazent, JAC will provide copies of all documentation
and and of case management summary report of youth/family progress to
Padre.
Padre Behavioral Hospital will provide:
• Select up to 25 youth participants to refer to JAC for case management. This
service is a voluntary service and the family has a right to decline service at
any time before the three month period.
• Meeting space for Case Managers to conduct contact visits with participants
referred to JAC by Padre.
Announcements of pending trainings and events.
Participation by designated staff in multi-disciplinary service pianning
meetings.

Indemnification:
Padre hereby agrees to hold harmless the City and its officers, employees, and
agents from all suits, actions, claims, or cost of any character, type, or
description brought or made on accoum of any injuries, death, or damage
received or sustained by any person or persons or property arising out of or
occasioned by any negligent acts of Padre or Padre personnel, if any, or its
agents or employees whether occurring during the performance of the services
hereunder or in the execution of the performance of any of its duties under this
Memorandum of Understanding.
Confidentially of Records of Individuals Served by this Agrcement:
• Padre and City agree to keep alI participant information confidential in
accordance with all applicable state and federal laws, statutes, and regulations
protecting the confiderniality of such information, including 42 CFR Part 2.
Insurance:
Padre agrees to provide certificate of insurance naming the City of Corpus Christi as an
additional insured, and a waiver of subrogation in favor of the City of Corpus Christi.
