BASIC SERVICES ITEM NO. 2 DESIGN PHASE shall be modified by additional services for the extension of an additional storm line of the Mediterranean drive offsite storm system farther into the Bayberry Terrace subdivision. Acting Assistant Director of Engineering Services __.. _ -- APR 2 2 2008 Apri122, 2008 7-05027 RE: Waldron Road -Purdue to Glenoak and Mediterranean Extension Project City Project No. 6274 Dear Jerry: During the final walkthrough of the Mediterranean Drive extension portion of the above referenced project, several notable discrepancies in the throat elevations of the post inlets for the offsite storm system were discovered. In order to define what deviations were made from the original design, we sent our survey crew to do a topographic survey. After the topographic survey was complete, the enclosed seven (7) sheets were drawn to show the construction deviations from design and provide recommended actions. A meeting with the contractor, the City's Construction Inspection Department and RVE, Inc. was conducted to specify construction deviations and recommended actions. The additional survey work and meeting conducted exceeds our scope of services required during the construction phase according to our contract. We re~ectfully request an_ amendment to our contract in the amount of_ $5,920 for_the additional survey work. drawings and meeting conducted. Now that we are at the end of this project and can see the necessary costs required for completion, this may be an appropriate time to consider extendng_an additional storm _ ------ line of the Mediterranean Drive offsite storm system farther into the Bayberry Terrace subdivision. The potential storm line would connect to the one of the newly constructed post inlets in the east Waldron Road ditch and extend along North and East Bayberry Place. Additional topographic surveying will be required to design this extension. The estimated additional construction cost is $244,500. The additional fee for the additional AMEND.
