Phase 1 (Project No. 2214) as authorized and administratively amended by: Ori final Small A/E A reement December 14, 2004 Administrative A royal $24,996.00 Amendment No. 1 Aril 19, 2005 Motion No. 2005-116 $239,906.00 Amendment No. 2 October 4, 2006 Motion No. 2005-116 $24,867.00 • In the original contract, Section 2. SCOPE OF WORK shall be modified by the addition of the following scope as further described in the attached Exhibit "A": Gilliam Street design (setup separate bid documents); hydraulic analysis of NW Crossing ditch system; visual inspection of McNorton Channels; and prepare design report addendum. • In the original contract, Section 3. FEES shall be modified as shown on the attached Exhibit "A" for a revised fee not to exceed $22,614.00 (Twenty-Two Thousand Six Hundred Fourteen Dollars and Zero Cents), for a total restated fee not to exceed $312,383.00 (Three Hundred All other terms and conditions of the Contract and Amendments between the City and Consultant will remain in full force and effect. (Maverick) is pleased to submit our 1'ce proposal for providing additional services for the McNortat Channel Improvements - Phase I (Project No. 2214) as outlined in the attached Man Hour Estimate. The additional scope of services and corresponding fees arc shown below: Scope of Services: 1. Separate the Gilliam Street hnprovements from the McNorton Chrumcl Improvement -Phase One bid package into separate a hid document. This will include the evaluation of the current Gilliam Street design to ensure conformance with in the cuncnt funds available (construction budget of $400,000). 2. Visually inspect the McNorton Channel basin for any minor construction or ntainicnancc corrections that would improve the capacity of this channel. 'I'bis would not include any surveying. Ifmry dents are identified to he added to the Gilliam Street bid package, Maverick will complete limited field measuranenis to determine estimated quantities. 3. Complete a hydraulic analysis of the drainage channel west of Rand-Morg:m Road that conveys flow from the NW Crossing subdivision to the McNorum Ditch "B:' This analysis would include nny impacts tmm the box culverts henenth Rnnd-Morgan Road. 'T'his analysis would he base) on existing city topographical data supplemented by visual observations by Maverick engineers but would not include any additional surveying work. Incorporate any recommended improvements in this section of ditch into the McNorton C'hanncl Improvements Design Report as an addendum to the original.
