The following paragraphs describe Amendment No. 5. The headings and outline designations refer to those
from the original contract. Amended sections retain the same number and letter designation; new sections
append the sequencing of the original contract.
^ EXHIBIT "A", PART 1. SGOPE OF SERVICES, SECTION B. BASIC SERVICES, ITEM NO. 4
CONSTRUCTION PHASE shall be modified to include thefollowing: The AlEwill perform contract
administration to provide construction administration for a total of 534 calendar days through
September 30, 2009.
^ EXHIBIT "A", PART 1. SCOPE OF SERVICES, SECTION B. ADDITIONAL SERVICES, ITEM NO.4
GONSTRUCTION OBSERVATION SERVICES shall be modified to include the following: Provide
construction observation for an additional 107 calendar days to accommodate project
construction time extensions for a total of 534 calendar days through September 30, 2009.
^ EXHIBIT "A", PART 1. SCOPE OF SERVICES, SECTION B. ADDITIONAL SERVICES, shall be
modified to include the following new ITEM N0. 9 EXTENSION OF CDNSTRUCTION
ADMINISTRATION AND OBSERVATION SERVICES: Provide additional construction administration
and cons#ruction observation services beyond September 30, 2009 when au#horized b the Ci to
accommodate project construction on a month-by-month basis.
