AMENDMENT N0.4
CONTRACT FOR PROFESSIONAL SERVICES
The City of Corpus Christi, Texas hereinafter called "City", and MARTINEZ. GUY 8~ MAYBIK,
INC. hereinafter called "Engineer", agree to the following amendments to the Contract for
Engineering Services for ADA Sidewalk Access Improvements alone Staples Street from
Leopard Street to Six Points (Project No. 6391), as authorized and administratively amended
by:

Exhibit "A" of the original Contract for Professional Services, Exhibit "A", Section 1.
Scope of Services, Part A. Basic Services Item No. 2 Design Phase Services shall be
amended as follows:
1. Perform and engineering investigation and analysis of the current drainage conditions
that the existing 6'x4' RCB culvert system services and substantiate its removal and
replacement with a new underground drainage system capable of handling the storm
event as per the City Storm Water Design Criteria. The results will be provided in a
engineering letter report that will include the study's findings (that will also contain a
discussion on the televising of the existing RBC and the discovery of the void condition
underneath the sidewalk segment adjacent to the KRIS Communication Building) and
present design recommendation as to the size culvert system that will replace the
existing dual 6'x4' RBC culvert.
2. After City's review and acceptance of the engineering report and per City's directive,
provide construction plans for a new underground drainage system along with new
manholes and special curb inlet structures that will be issued for construction to the
Staples ADA project Contractor. The plans will also provide traffic centrol plans to
facilitate removal of the existing RBC culvert and the installation of the new underground
drainage system.
Exhibit "A" of the original Contract for Professional Services, Exhibit "A", Section 1.
Scope of Services, Part A. Basic Services Item No. 4 Construction Phase Services shall
be amended as follows:
1. Perform additional construction administration support services and day-today
inspections of the proposed underground drainage conduit installation.
Exhibit "A", Section 3. Fees, Part B. Summary of Fees shall be modified as shown on the
attached Exhibit "B" for a revised fee not to exceed 57.680.00 (Seven Thousand Six Hundred
Eis~hty Dollars and Zero Cents), for a total restated fee not to exceed 5589,553.00 (Five
Hundred Eichty-Nine Thousand Five Hundred Fifty-Three Dollars and Zero Cents).
All other terms and conditions of the March 8, 2005 contract between the City and Engineer,
and of any amendments to that contract, which are not specifically addressed herein shall
remain in full force and effect.

Re Proposal To Provide Professional Engineering Services in Connection with the Dual 6'x4'
Reinforced Concrete Box (RCB) Culverts at Staples near the Agnes Intersection in front
of the KRIS Communication Building, Corpus Christi, Texas

We aze pleased to submit this proposal to the City of Corpus Christi, Texas (hereafter referred to
as Client) to provide our engineering services for the evaluation and preparation of construction
plans for the removal and replacement of the dua16'x4' RCB culvert that is associated with
Staples ADA improvements project, which is currently under construction. The scope of
services shall include the following:

We propose to perform the previously defined Basic Scope of Services for a fixed fee of
SEVEN THOUSAND SIX HUNDRED EIGHTY DOLLARS AND NO CENTS
($7,680.00). Please be advised that this proposal considers providing any other service from
those described in the Basic Scope of Services to be Additional Services.
Any Additional Services performed by our company, as authorized by Client, would be
charged according to the following rates: Project Engineering and Management at
$105/hour; 2 Man Survey Crew at $100/hour; 3 Man Survey Crew at $125/hour; Registered
Professional Land Surveyor at $95/hour; Survey Technician at $65/hour; Field Inspector at
$75/hour and Secretarial at $48/hour
The determination of our compensation has been made on the basis of prompt payment of our
invoices and the orderly and continuous progress of the project.
This proposal represents the entire understanding between Client and our firm and may only
be modified in writing by the undersigning parties involved in this agreement. If it
satisfactorily sets forth your understanding of our agreement, we would appreciate you
signing a copy of this proposal and returning it to our office. This proposal will be open for
acceptance for a period of thirty (30) calendar days from this date unless changed by us in
writing.
