The proposed amendment is for Stage Z Phase 2, which consists of the re-submittal of plans, specifications and contract documents for Helen and Theresa Streets. This project was originally submitted in November 2004, but was not bid nor constructed due to funding. The total fee for Amendment No. 2 is $13,100.00. Including this amendment the revised fee for basic services is 6.0% of the estimated construction costs. The originat design phase of this. project was ..:. included in Amendment No...l alongwith the bidding and construction phases. . It is our understanding that this project will be bid and constructed as part of another Bond 2008 project. Therefore, construction observation is not included in this amendment. The attachments included in this submittal are Exhibit A-Scope of Services, Exhibit B-Summary of Fees, Consultant Fee Schedule and a Cash FIow Invoice Schedule. Basic Services 2A. Re-submittal of Final Plans. Consists of providing QA/QC measurees to update the Final Submittal to the City from 2004. The scope of work includes the following: a. Updating ADA Ramps on the project to current ADA &TDLR standards. Re-submit to TDLR for review/acceptance. b. Updating final quantities and construction estimate on the project. c. Providing QA/QC for utility tie-in points for storm sewer box culvert, wastewater line and water line. This includes taking a look at any potential conflicts with existing utilities such as Gas, AT&T and AEP. d. Provide an updated set of contract documents and specifications e. Provide an updated project schedule f. Provide an updated schedule for construction including traffic control and sequencing. g. Provide site visits to re-evaluate existing conditions for driveways, sidewalks, fences and utilities. h. Provide one (1) set of Final plans, specs and contract documents, estimate and project schedule to the City. i. Attend meeting with the operating department as required. 2. N0.2 EXHIBIT "A" Page 2 of 3 AREA STREET AND DRAINAGE IMPROVEMENTS PHASE 26 -HELEN & THERESA ST. PROJECT N0.6169 AMENDMENT N0.2 The following items are changes or revisions from the previous 2004 submittal: 1. Plans a. Roadway/Sidewalk/ADA/Driveway i. Changed the proposed pavement section from lime treated sub-grade to geo-grid and limestone flexible base. ii. Re-design on ADA ramps to match Ped-05 standards. Will need to re-submit to TDLR for review. iii. Site visit to verify existing conditions for fences, driveways sidewalks etc. b. Water Line i. Provide C~4/QC for design of water line & any existing utility conflicts. ii. Revised tie-in for water line at Gollihar and Sunnybrook c. Wastewater Line i. Changed the design to pipe bursting instead of laying a new parallel line with new manholes. This change was based on the experience from the Mansheim project for the identical line size. ii. Verified tie-in points & flow lines for existing manholes. d. Drainage i. Verified tie-in points & flow lines for Gollihar &Sunnybrook locations. ii. QA/QC for potential conflicts with existing utilities. e. General i. Contacted utility companies to verify locations of their existing utilities to identify potential conflicts (AEP, Gas Dept., AT&T, Time Warner & Grandej. 2. Contract Docs a. Provide QA/QC for existing contract dots & front end dots. Updated changes in City personnel for signature b. Update items such as Notice to Contractor's-A. 3. Specifications a. Provide QA/QC for existing specifications b. Added specifications for new items such as pipe bursting 4. Estimate a. The estimate has to be updated to reflect the estimated construction cost for 2009 dollars. b. Quantities had to be revised based on changes in design for utilities and roadway items. 5. Topographic Survey a. The survey crew had to pick up additional topo for fences that had been installed or replaced since the last submittal. For example, the property at the intersection of Helen/Theresa St. which installed a wooden fence within the City right-of-way. At this location it appears that there is also a storage building that is within the City right-of--way.. b. The survey crew also had to verify tie-in points for the storm sewer box on Gollihar and also flow lines for the wastewa#er line. Page 1 of 1 AMEND.
