(BOND 2008) (Project No. 6490), as authorized and amended by: Ori final Contract Aril 21, 2009 Motion No. 2009-095 $1,414,850.00 Amendment No. 1 November 17, 2009 Motion No. 2009-328 $92,900.00 EXHIBIT "A", PART 1. SCOPE OF SERVICES, SECTION B. ADDITIONAL SERVICES, shall be amended to include the following Additional Service: ITEM N0.8 RTA BUS STOPS (Authorized): - Design of the Regional Transportation Authority's (RTA) bus stop pedestrian pads for a total additional fee in the amount of $6,720.00. • Phase 1 - 4 bus stop pedestrian pads ($3,840.00) • Phase 2 - 3 bus stop pedestrian pads ($2,880.00) EXHIBIT "A" PART 3. FEES SECTION C. SUMMARY OF FEES shall be modified and is attached as Exhibit "A" for a revised fee not to exceed ~i720.00 (Six Thousand Seven Hundred Twenty Dollars and Zero Centsl, for a total restated fee not to exceed $1,514,470.00 (One Million Five Hundred Fourteen Thousand Four Hundred Seventy Dollars and Zero Cents). (Project No. 6490) AMENDMENT N0.2 Dear Mr. Anaya: Please accept this letter as our proposal to amend our original contract to obtain additional funding for the design of the Regional Transportation Authority's (RTA) bus stop pedestrian pads. There are eight (8) existing bus stop pedestrian pads spread throughout Kostoryz Road, Phases 1 & 2, one (1) of which will be removed and seven (7) that will need to relocated and/or rebuilt as part of the Bond 2008 Kostoryz Road Improvements Project. The current contract has no allowance for these facilities because the RTA had originally planned to rebuild the bus stop pedestrian pads themselves. After negotiations between City staff, GE CH2M HILL personnel and RTA officials, it was concluded that the more feasible approach would be to include the bus stop pedestrian pad upgrades as part of the Bond 2008 Kostoryz Road Improvements Project, which is due to begin Phase 1 & 2 construction in early 2010. The proposed RTA bus stop pedestrian pad construction will consist of the following: • Demolition of existing bus stop pedestrian pad facilities • New concrete bus stop pedestrian pads • New ADA compliant ramps, landings, and sidewalks • New detectable warnings AMD. February 11, 2010 The estimated construction cost for each proposed bus stop pedestrian pad is $12,000. This estimated cost does not include any type of shelter structure. A total additional engineering fee of $6,720 will be needed for the design, RTA coordination, and assemblage of federal boiler plate documentation for seven (7) bus stop pedestrian pads that are within the Kostoryz Road Improvements, Phases 1 & 2 Project. Funds will be allocated to Phases 1 & 2 as follows: • Phase 1 -four (4) bus stop pedestrian pads = $3,840 • Phase 2 -three (3) bus stop pedestrian pads = $2,880 The following are the proposed changes to the original amended, executed contract. 1.
