INC., hereinafter called "CONSULTANT," agree tv the following amendment to the Contract for Professional Services for Street Dverla s Grvu 1 - Bond 2fll}8 Prv'ect Nv. fi475 as authorized and administratively amended by: Original Contract February 14, 2QD9 Motion No. 2DD9-D3D $549,83D.DD EXHIBIT A, SECTION 1. SCOPE OF SERVICES, PART B. ADDITIONAL SERVICES, shall be modified by the additional paragraphs shown below. 5. TARLTON STREET SIDEWALK AND ^RIVEWAY REPLACEMENT. Provide design, construction drawings and proposal for the removal and replacement of approximately 11,DDD square feet of sidewalk and 5,6DD square feet of driveways on Tarlton Street between Churchill ^rive to Ayers Street. The design shall include preparation of plan sheets for inclusion in the change order to the contract, preparation of a proposal form, composition of necessary additional specifications, and preparation of a preliminary opinion of probable cos#. Submit a set of change order plans and documents to the City for review. Receive comments, make changes as necessary. The work will be accomplished by change order to the existing Overlays Group 1 Contract. Submit final documents to the City. Meet with the City and the contractor regarding the change order. Work with tl~e City and the contractor to finalize the change order and if necessary, attend a preconstruction conference for the change order work. Make site visits as required to coordinate with the City construction inspector and the contractor. fi. TARLTGN STREET SIDEWALK AN^ DRIVEWAY TOPOGRAPHIC SURVEY. Provide field surveys far sidewalk and driveway design on Taritan Street between Churchill Drive and Ayers Street, including the necessary control points, coordinates and elevations of paints. Establish base survey control for line and elevation staking. All work will be tied to and conform with the City's Global Positioning System control network and comply with Category 6, condition I specifications of the Texas Society of Professional Surveyors' Manual of Practice for Land Surveying in the State of Texas, Ninth Edition, and will include reference to a minimum of two found boundary monuments from the project area. 7. TARLT~N STREET RIGHT OF WAY fROW] SURVEY. Review existing ROW and easements tv ascer#ain any conflicts and provide field ROW survey of Tarlton Street from Churchill Drive to Ayers Street. All work will comply with Category 1-A, Condition specifications of the Texas Society of Professional Surveyors' Manual of Practice for Land Surveying in the State of Texas, Ninth Edition. All work will be tied to and cvnfvrm with the City's Global Positioning System control network and comply with a1! TxDOT requirements as applicable. AlE Consultant will perform all necessary deed research. The City will provide title commitments far each parcel in question, which will be used as the basis for the AIE deed research.
