BASIC SERVICES, shall be amended as shown in the attached Amendment No. 21 Exhibit "A"and summarized below forthe following project: Padre Island Elevated Storage Tank {Project Na._8551) - Item No. 4.1 Construction Phase shall be amended to include additional Construction Phase Services (10 hours per week}. In the original contract, EXHIBIT "A" SECTION 1. SCOPE OF SERVICES, PART B. ADDITIONAL SERVICES, shall be amended as shown in the attached Amendment No. 21 Exhibit "A"and summarized below for the following project: Padre Island Elevated Stora a Tank Pro'ect No, 8551 - Item No. 5.1 Construction Observation Services shall be amended to include additional Construction Observation Services (20 hours per week). In the original contract, EXHIBIT "A," SECTION 3. FEES, PART C. FOR TOMORROW Re: PADRE !BLAND ELEVATED STORAGE TANK, PART A (Project No. 8557) Dear Mr. Anaya: In respanse to City request, we are proposing the following small AfE contract for the Padre Island Elevated Storage Tank ('1.5MG) project. Project Description: The City of Corpus Christi, Texas, hereinafter called "City", and LNV Engineering, Inc. Hereinafter called "Engineer", agree to the following Contract for Construction Phase Services and Construction Observation Services associated with time extensions from Contractor Change Orders No. 1-3, in the amount of 319 calendar days. These services were not included as park of the original scope. Project Background: The Padre island Elevated Storage Tank--Park A project was initiated through Amendment No. 5 to LNV, Inc_'s contract for professional services for the Water System Interactive Hydraulic Analysis (Project No. 8487) Project as Task No. 37, on December 25, 20Q5. During the course of the Construction Contract work by Landmark Structures I, L.P., the City elected to modify the Contract, with the issuance n€ three change orders, as follows. Change Order No. 1 consisted of Pile integrity Testing (PIT} and resulted in a time extension to the Contractor of (5} calendar days. Change Order No. 2 involved the addition of cable couplings, cable brackets, and inspection rail and. rigging inside of the tank pedestal and access tube to accommodate the cabling associa#ed wifh multiple antennae to be installed on the tank pedestal and roof top, as requested by the City's MIS and Police Departments. Change Order No. 2 resulted in a time extension to the Contractor in the amount of (42) calendar days. The revised contract completion time trough Change Order No. 2 was December 'l9, 20Q9. AMD. N0.21 EXHIBkT "A" Pa e 1 of 5 Bp'i NAVIGATION I Si11TE 3p0 I CORPUS CHRISTI, l'EXAS 7E OFFICE 361.883.1484 I Fax 357.883.1986 I WWW.LIVVINC,GOM In December, 2009, the City requested that LNV, Inc. provide professional services to modify the contract documents to include sleeves in the new Padre Island Elevated Storage Tank's pedestal wall, inlet/outlet piping modifications required to allow for future chemical boosting facilities, electrical modifications for City-network communications, police and #ire communications and access control, SCADA modifications to provide control and communication with the Sand Dollar Purnp Station, as well as landscaping, irrigation, and fencing improvements for the Tank site and utility easement. This was accomplished through Amendment No. 20 that was approved and signed on April 1, 2010. LNV then prepared Change Order No. 3, which required the Contractor to provide the above modifications and resulted in a time extension for the Contractor in the amount of {272) calendar days. Change Order No. 3 was signed on April 22, 2090. The Contractor resumed work on the project vn June 7, 2010. LNV Engineering, Inc. proposes to provide an addi#ionai (95) weeks of Construction Phase Services and Construction Observation Services for the period from June 7, 2010 through the revised Contract Completion Date of September 17, 2090. Scope of Services: The Scope ofi Services for this contract is divided in#o two #asks: - Task 1 -Basic Services~l0 hours per_week). Provide Construction Phase Services. - Task 2 -Additional Services f20 hours per week). Provide Construction Observation Services. Breakdown of Services: - Task 1 -~ Basic Services -Construction Phase. The A/E will perform contract administration to include the following: • Participate in monthly construction status meetings and provide a recommended agenda for critical construe#ion activities and elements impacted the project. • Review for conformance to contract documents, shop and working drawings, materials and other submittals. • Review field and laboratory tests. • Provide interpretations and clarifications of the contract documents for the contractor and authorize required changes, which do not affect the contractor's price and are not contrary to the general interest of the City under the contract. • Make regular visits to the site of the Project to confer with the City project inspector and contractor to observe the general progress and quality of work, and to determine, in general, if the work is being done in accordance with the AMD. NO. 21 1=XHIBIT "A" Pa a 2of5 contract documents. This will not be confused with the project representative observation or continuous monitoring of the progress of construction. • Prepare change orders as au#horized by the City {coordinate with the City's constnac#ion division); provide interpretations and clarifications of the plans and specifications for the contractor and authorize minor changes which do not affect the contractor's price and are not contrary to the general interest of the City under the contract. • Make final inspection with City staff and provide the City with a Certificate of Completion for the project. • As applicable, review and assure compliance with plans and specifications, the preparation of operating and maintenance manuals (by the Contractor) for al! equipment installed on this Project. These manuals will be in a "multimedia format" suitable for viewing with Microsoft's Internet Explorer, version 3.0. As a minimum the Introduction, Table of Contents, and index will be in HTML (HyperText Markup Language) format, with HyperText links to the other parts of the manual. The remainder of the manual can be scanned images or a mixture of scanned images and text. use the common formats far scanned images - GIF, TIFF, JPEG, etc.. Confirm before delivery of the manuals that all scanned image formats are compatible with the image-viewing software available on the City's computer -Imaging for Win95 (Wang) and Microsoft Imaging Composer. Deliver the manuals on a CD-RDM, not on floppy disks. • Review construction "red-line" drawings, prepare record drawings of the Project as constructed (from the "red-line" drawings, inspection, and the contractor provided plans) and deliver to the Engineering Services a reproducible set and electronic file (AutoCAD r.14 or later) of the record drawings. All drawings will be CADD drawn using dwg format in AutoCAD, and graphics data will be in dxf format with each layer being provided in a separate file. Attribute data will be provided in ASCII format in tabular form. All electronic data will be compatible with the City GIS system. The City staff will: • Prepare applications/estimates for payments to contractor, • Conduct the 1•rnal acceptance inspec#ion with the Engineer. - Task 2 -Additional Services • Provide a project representative {PR} #o provide periodic construction inspection. • Through such additions! observations of Contractor's work in progress and feld checks of materials and equipment by the PR and assistants, the A!E shall endeavor to provide further protection for the CITY against defects and deficiencies in the Work, to include the following: AMD. N0.21 EXHIBIT "A" Pa e3of5 • Confer with City's Construction Inspector and Contractor #o observe the progress and quality of work, and to determine, in general, whether the work is accordance with project design. • Review and approve all contractor's shop drawings and/or submittals. • Coordinate necessary change orders during the process of the work. City will prepare and process all change orders, • Review and approve all proposed layouts, lines, levels, and elevations prior to installation or construction to assure project design and intent. The fee wilt be full and total compensation for services and for all expenses incurred in perForming these services. Engineer will submit mon#hly statements for basic services rendered. Each statement will be based on the project percent completion at the time of billing. City will make prompt monthly payments in response to Engineer's monthly statements. B. Summary of fees.
