N. Stevens Water Treatment Plant Process Monitoring Instrumentation and Automation Imp rovements Pro'ect No. 8600) as authorized and amended by: Original Small AIE June 3, 2008 Administrative Approval $47,000.00 Amendment No. 1 April 21, 2009 M2009 -097 $398,600.00 Amendment No. 2 January 19, 2010 M2010 -023 $380,915.00 Amendment No. 3 July 26, 2010 Administrative Approval $47,550.00 EXHIBIT "A" PART 1. SCOPE OF SERVICES SECTION B. ADDITIONAL SERVICES, shall be amended to expand and/or add the following Additional Services as set forth in Exhibit "A ": • ITEM NO. 12 Change Order No. 1 shall be amended to include in Additional Services — $12,565.00 • ITEM NO. 13 Filter PLC Upgrade Package Modifications shall be amended to include in Additional Services — $26,040.00 EXHIBIT "A" PART 3. STEVENS PROCESS MONITORING INSTRUMENTATION AND AUTOMATION IMPROVEMENTS: PROJECT NO. 8600: FILTERS PLC UPGRADE Dear, Mr. Green This letter is our request for additional funding of $38,605 for Amendment No. 4 to the ONSWTP Project 8600, This Amendment is for additional Contract Engineering Services for modification of the Bath Engineering SOW to facilitate the following changes: Instrumentation and Automation improvements Project (8600) Change Order #1: - Relocate new sample pump to move sample stream from the new vault to the Analyzer Shed 1201 - Add piping, conduit, and wiring for the sample pump - Tie existing ammonia analyzer and. new turbidity analyzer into the new sample stream - Do not install sample pumps SP1201 -A and SP1201 -B and associated power wiring and switches - Demolish existing sample piping from these two existing points to Analyzer Shed 1201. - Install new 24 -fiber optic cable from PCR #2 to the WashWater Pump House with all necessary cable patch panels and terminations - Additional construction support and coordination Filter PLC Upgrade Project (8600, Amendment No.2): - Redesign PLC panel wiring to allow for 120VAC digital 1/0 in lieu of 24VDC digital 1/0 on twenty -two (22) Filter PLC panels, one (1) Wash Water Pump Station PLC panel, and one (1) Wash Water Tower PLC panel. - Add grade -level operator stations for 12 Backwash, 12 Air Scour and 12 Filter to Waste valves for the North Filters. - Add grade -level operator stations for 10 Backwash and 10 Filter to Waste valves for the South Filters. - Field verification of existing wiring configurations of valves requiring grade -level control stations. - Redesign all North and South PLC enclosure mounting. - Revise current engineering documents for all items listed above. - Additional construction support and coordination allowing for City Water Department personnel Project construction- - Items listed in this section based on meeting between Bath and City Water Dept. personnel on 28 October 2010.
