The City of Corpus Christi (the "City') and Southwest Medical Strategies d.b.a. The Doctors' Center (the
"Contractor ") do hereby agree to amend that certain Occupational Medical Services Service Agreement
Number SA08 -038 (the Agreement).
I,
The City and the Contractor do hereby mutually agree to exercise the second and last of
two twelve -month extension options. Therefore, the Agreement is hereby extended from
November 1, 2011 and continuing through October 31, 2012.
"ALL OTHER TERMS AND CONDITIONS REMAIN UNCHANGED"
IN WITNESS WHEREOF, the parties hereto have affixed their signature.
