Stevens Water Treatment Plant _Facility Alterations (Project No. 8657} as authorized and administratively amended by: Original Contract i i June 9, 2009 Motion No. M2009 -161 $165,700.00 Amendment No. 1 December 4, 2010 Administrative Approval $46,903.00 Amendment No. 2 July 18, 2011 Administrative Approval $48,755.00 Amendment No. 3 October 26, 2011 Administrative Approval $7,500.00 Amendment No. 4 May 14, 2012 Administrative Approval $19,315.00 From the original contract, Exhibit "A ", Additional Services, Item No. 4 Construction Observation Services is amended to include additional Construction Observation Services as specified in Amendment No. 5 Exhibit "A" and as follows: + Increase construction observation site visits from one per month to one per week • Increase progress meeting attendance from one per month to two -four per month, depending on phase of construction + Increase number of months of construction observation by three months From the original contract, Exhibit "A ", Summary of Fees shall be modified as specified in Amendment No. 5 Exhibit "B" Summary of Fees, for a fee not to exceed $31,625.00 (Thirty One Thousand Six Hundred Twenty Five Dollars and Zero Cents) for a total restated fee not to exceed $319 (Three Hundred Nineteen Thousand Seven Hundred Ninety Eight Dollars and Zero Cents.
