AMENDMENT NO 5 FOR AIRLINE ROAD — SARATOGA BLVD TO RODD
FIELD ROAD (CITY PROJECT NO. 6465)
Dear Mr. Giles:
The original Exhibit A of the scope of services for Airline Road from Saratoga to Roddfield
Road included construction administration phase services to be performed by HDR. One of the
items listed in the scope of the construction administration phase services is to make site visits to
the project estimated at 78 weeks for a maximum, of 3 hours per week: The intent of the
construction administration phase site visits is to obtain intermittent construction progress
updates, log the contractor's progress, and collect pertinent photographs of the project.
Beginning the week of January 9, 2012 the City construction inspection staff beg an requesting
HDR to make additional site visits to help coordinate and mediate construction conflicts with
AT &T conduits, City water lines, and property owner access concerns. As of June 30, 2012 the
additional costs HDR has incurred beyond the original construction administration phase scope is
$5,353:00. If the City continues to need additional field hours from HDR for the remaining
duration of weeks in HDR's scope of services, the projected cost for this effort using January to
June as a basis will be $10,000.00 for a total amendment fee of $15,353.00.
To that end, we propose an amendment to our contract. Below is a summary of the proposed
additional. fees.
