LLC hereinafter called "Engineer", agree to the following amendments to the Contract for Professional Services for O.N. Stevens Water Treatment Plant Facility Alterations (Project No. 8657 as authorized and administratively amended by: Original Contract June 9, 2009 Motion No. M2009 -161 $165,700.00 Amendment No. 1 December 4, 2010 Administrative Approval $46,903.00 Amendment No. 2 July 18, 2011 Administrative Approval $48,755.00 Amendment No. 3 October 26, 2011 Administrative Approval $7,500.00 Amendment No 4 May 14, 2012 1 Administrative Approval $19,315.00 Amendment No. 5 August 21, 2012 Administrative Approval $31,625.00 From the original contract, Exhibit "A", Basic Services, Item No. 2 Design Phase and Exhibit 'W', Additional Services, Item No. 4 Construction Observation Services are amended as specified in Amendment No. 6 Exhibit "A" and as follows: • Increase meeting attendance for security issues, revise drawings numerous times; and • Increase scope for construction observation. From the original contract, Exhibit "A ", Summary of Fees shall be modified as specified in Amendment No. 6 Exhibit "B" Summary of Fees, for a fee not to exceed $10,180.00 (Ten Thousand One Hundred Eighty Dollars and Zero Cents) for a total restated fee not to exceed $329,978.00 (Three Hundred Twenty Nine Thousand Nine Hundred Seventy Eight Dollars and Zero Cents All other terms and conditions of the June 9, 2009 contract between the City and Consultant, and of any amendments to that contract, which are not specifically addressed herein shall rerpain in full force and effect.
