Scope of Services, Part A. Basic Services, Item No. 2. Design Phase shall be amended as set forth in the attached AMD No. 2, Exhibit "A." In the Original Contract, Exhibit "A", Section III. Fees, Summary of Fees shall be amended for a fee not to exceed $39,154.00 (Thirty Nine Thousand One Hundred Fifty Four Dollars Zero Cents), for a total restated fee not to exceed $819,721.00 (Eight Hundred Nineteen Thousand Seven Hundred Twenty One Dollars and Zero Cents). Monthly invoice will be submitted in accordance with Exhibit "B". All other terms and conditions of the January 29, 2013 contract between the City and Engineer, and of any amendments to that contract, which are not specifically addressed herein shall remain in full force and effect. (FNI) would respectfully request your consideration of our proposed amendment#2 for South Staples Street from Brawner Parkway to Kostoryz Road (City Project No. E12095). The amendment includes additional design services and coordination with the City's Gas Department for complete replacement of their facilities within the project. The amendment also includes additional design services necessary for improvements at Brawner Parkway. FNI respectfully requests a contract amendment in the amount of$39,154 to address the additional time and effort required to support these project tasks.
