SUBJECT: Contract Amendment One
Staples Street—Morgan Avenue to IH 37 (Antelope Street
COCC Project No. E12096
Dear Mr. Harris:
Enclosed for your review and consideration is the proposed Contract Amendment One for the
referenced project. The contract amendment adds additional SUE services for identifying the
limits of an existing concrete base material. Our research and the Geotechnical bores confirm
the existence of an existing concrete base material in the section of Roadway from Kinney
Street to Morgan Avenue approximately 4100 LF long and at least 60' wide. In order to
accurately confirm the limits of existing concrete pavement both longitudinally and horizontally
we are proposing to utilize Ground Penetrating Radar (GPR). The information will allow us to
determine whether any of the concrete is underneath the Curb & Gutter and will allow us to
provide more detailed information to the contractors for the demolition process.
We proposed to complete these services at a fee not to exceed $11,000 for a total restated fee
of $848,162.50. We propose to utilize the services of SAM Surveying and Mapping, LLC to
complete the GPR work. Included are executed copies of the applicable City standard forms.
We thank you for your consideration and approval. Should you have any questions, please do
not hesitate to call.
Very Truly Yours,
NAISMITH RING, INC.
