Barge Dock Improvements City Project No: 3426 Contract Amendment No. 1 —Professional Engineering Services LJA Project No. C007-12040 Dear Mr. Torno: I. SCOPE OF WORK As requested, we have prepared Contract Amendment No. 1 (CA1) to remove Additional Services - Project Definition and to create Basic Services. The purpose of CA1 is to reallocate unused existing contract funds to provide additional funds for design, bid, and construction phase services for City-requested barge dock improvements. The net result of CA1 is a zero- dollar change to the Original Contract. The proposed changes to the Original Contract are as follows: 1. Increase Basic Services: Preliminary Phase scope of services to perform a site visit, prepare a scope of work, and opinion of probable construction cost for recommended improvements and meet with City for final approval; 2. Increase Basic Services: Design Phase scope of services to create plans and contract documents for bidding and construction; 3. Increase Basic Services: Bid Phase scope of services to provide Bid Phase Services; 4. Increase Basic Services: Construction Phase scope of services to provide Construction Phase Services; and 5. Remove Additional Services: Project Definition. The need for CA1 arose from the City's request to LJA to recommend repairs to the Barge Dock, to design construction documents for those repairs, and to provide bid and construction phase services. CONTRACT CONDITIONS Preliminary Phase Basic Services as described in the original Exhibit "A" are hereby removed and replaced with the following services: • Perform a site visit to quantify scope: Recommended improvements are a result of the investigation of findings report dated 4/27/16. Improvements included patch, fill, and seal of concrete within the paving, along the bulkhead, and on top of the seawall; fill void below the seawall; repair void around stormwater outfall RCP pipe; seal concrete paving adjacent to bulkhead to protect against saltwater attacking; and parking lot striping. • Prepare a scope, exhibit and opinion of probable construction cost for recommended improvements. • Meet with City staff to discuss and finalize the scope of improvements. Design Phase Basic Services as described in the original Exhibit "A" are hereby removed and replaced with the following services: • Prepare construction drawings and contract documents with specifications for repairs agreed upon during preliminary phase. • Prepare and submit 100% review package. • Attend review meeting with the City. • Revise construction documents per comments received at the 100% review meeting. • Prepare and submit final sealed plans and contract documents for bidding. Bid Phase Basic Services shall be provided in accordance with Original Contract, Exhibit "A" excluding Item e). Construction Phase Basic Services shall be provided in accordance with Original Contract for an estimated 3-month duration. If construction extends beyond three (3) months, additional fees will be required. Improvements do not include repairs to timber piles, sheet piles, tiebacks, and storm sewer pipes. The scope of work of this Contract Amendment is limited to the above-described work only. Any engineering efforts requested outside of the stated scope of work will require another amendment to the contract and additional fees. II. BASIS OF COMPENSATION &SCHEDULE OF PAYMENT LJA proposes to perform the above-described scope of work for a zero-dollar change to the Original Contract. The additionally-required fees are as follows: - Preliminary Phase $ 6,545.88 - Design Phase $ 29,267.41 - Bid Phase $ 8,401.14 - Construction Phase $ 15,170.86 - Total Additional Fees $ 59,385.29 To create the funds to cover the additionally-required fees, a portion of the AMD NO. 1 approved fees for Additional Services: Project Definition ($59,385.29) will be EXHIBIT'A' Page 2 of 4 • Mr.Shane Torno,P.E. Barge Dock Improvements June 23,2016 Contract Amend.No. 1 (City Project No: 3426) reallocated, thereby creating a zero-dollar change contract amendment as shown in the attached Summary of Fees. Invoices shall be prepared, submitted and paid on a monthly basis in accordance with the Original Contract. Invoices will be based upon engineering efforts spent at the time of invoicing.
