Rodd Field Expansion—Project No. El 5112 Contract Amendment No. 5—Basic Services (Design Phase) LJA Project No. C007-15025 Dear Mr. Hale, 1. SCOPE OF WORK We have prepared Contract Amendment No. 5(CA5)to our Rodd Field Expansion NE Contract to cover additionally requested Design Phase and Additional Services engineering efforts to: Basic Services (Design Phase) A. Investigate and design a wastewater service line to serve the Calvary Chapel building. This additional work will include surveying efforts along with alterations to the construction plans. B. Adjust storm sewer system per Utility Department's new direction to adhere to the MC27 design. This additional request and MC27 design was only recently provided to us. We have completed the storm water design and submitted two iterations of the Engineering Letter Report showing the proposed storm sewer alignment. To revise the system(s) as requested will require substantial changes to the construction plans including roadway grades,storm water pipe locations and the addition of an outfall structure at both proposed outfalls. C. Coordination and redesign efforts necessary to accommodate a proposed development at the Rodd Field and Yorktown intersection that was recently provided by City Staff. This work included the addition of driveways, deceleration lanes, relocation of storm sewer, etc. D. Revise street plan sheets to include a deceleration lane adjacent to the Kolda Elementary School. E. Update construction cost estimate as well as bid form with new quantities resulting from the above mentioned changes. AMEND. II. CONTRACT CONDITIONS LJA will perform the above-described work for the fees established below. My engineering efforts requested outside of the stated scope of work will require another amendment to the contract end additional fees. • III. BASIS OF COMPENSATION LJA proposes to perform the above-described scope of work for a Lump Sum fee of 539,373.18 as shown on the attached Manhour Breakdown for Services by Task, Exhibit B, and summarized in the table below.
