("Consultant"), agree to the following amendments to the Contract for Professional Services for Yorktown Boulevard from Everhart Road to Staples Street BOND 2014 (Project No. E13096): Original Contract February 18, 2014 Ordinance No. 030095 $990,929.00 Amendment No. 1 February 3, 2015 Administrative Approval $0.00 Amendment No. 2 August 2, 2016 Administrative Approval $35,405.00 In the Original Contract, Exhibit "A", Section I. Scope of Services shall be amended as set forth in the attached Amendment No. 3, Exhibit"A". In the Original Contract, Section III shall be amended based on the modified scope of services in the attached Amendment No. 3, Exhibit "A" for a fee not to exceed $20,818.00, for a total restated fee not to exceed $1,047,152.00. All other terms and conditions of the February 18, 2014 Contract for Professional Services between the City and Consultant,and of any amendments to that contract,which are not specifically addressed herein shall remain in full force and effect. Box 9277 Corpus Christi,TX 78469-9277 Re: Yorktown Boulevard—Everhart Road to Staples Street(Bond 2014) Project No. E13096 Amendment No. 3 Dear Mr. Edmonds, Freese and Nichols, Inc. (FNI)would respectfully request your consideration of our proposed Amendment#3 for Yorktown Boulevard—Everhart Road to Staples Street(City Project No. E13096). This amendment is requesting approval to compensate FNI for additional services related to the additional repairs needed in the Crossgate Ditch Culverts. The amendment includes additional construction phase services to make site visits to inspect repair efforts including assessment of concrete spalling, inspection of concrete surfaces after chipping for removal of deteriorated concrete, inspection of placement of additional reinforcing added to replace corroded reinforcing, review of sandblasted and cleaned surfaces prior to application of repair material, measurement and assessment of crack repairs, and coordination with City inspector, contractor,and subcontractor regarding repair methods and payment. The condition of the 5 box culverts(5'x8' cast in place boxes)were assessed using video footage during the project design phase. When the construction phase commenced the boxes were drained to allow better access. As the repair of the culverts commenced it became apparent that the extent of the deterioration of the concrete exceeded the original estimates for repair. A Change Order with the Contractor has been prepared and executed. This amendment will allow FNI to assist the City in administration of the work associated with that Change Order. Freese and Nichols, Inc. respectfully requests a contract amendment in the amount of$20,818.00 to address the aforementioned items. A. Basic Services Construction Phase 1.
