Lab shall perform services on the project described below as provided in this Task Order and in the Agreement. This Task Order shall not be binding until it has been properly signed by both parties. Upon execution, this Task Order shall supplement the Agreement as it pertains to the project described below. COMPENSATION —The total amount not to exceed for this Task Order only is$112,500.00. This Task Order is approved and Lab may proceed. (PSI) is pleased to have been selected based on qualifications to perform Construction Material Testing and Observations Services for the Residential Street Rebuild Program (E17019). We understand Bid Part 1 will consist of approximately 14 streets and Bid Part 2 will consist of approximately 18 streets that will be subjected to pavement rehabilitation or pavement reconstruction. Approximately 33,300 lineal feet of roadways are included in this project. The estimated fee for construction materials testing services was determined on a unit rate basis in accordance with PSI's 2018/2019 Master Services Agreement for Geotechnical and Materials Testing (Project No. 17099A). Based on the anticipated construction materials testing services, the estimated fee for Bid Part 1 is $50,020.00, and the estimated fee for Bid Part 2 is $62,480.00. The total fee for both Bid Parts 1 and 2 is $112,500.00. A breakdown of the anticipated construction materials testing services and fees for Bid Part 1 and Bid Part 2 are presented in the attached Estimated Fees. This proposal includes our services to observe, test and report on the construction material placement activities. Please note that our services will be performed on an intermittent, call-out, as requested basis. This proposal does not include full time or continuous on-site observation services. Scheduling will be the responsibility of our client's designated representative. Requests for services must be coordinated prior to the close of the business day preceding the requested activity.A project management fee or a surcharge on the technician's hourly rate may be assessed if the scheduling is not performed on the preceding day. Please note that the fees for our services are directly related to the contractor's schedule, the quantity of service requests, the quantity and frequency of failed tests/ retests, construction delays and other issues outside the control of PSI. We will work with the client's designated representative to control the budget for our services; however, invoices will be based on the actual service requests.
