CONTRACT FOR PROFESSIONAL SERVICES
AMENDMENT NO. 2
The City of Corpus Christi, Texas, a Texas home-rule municipal corporation ("City") and LNV, Inc., agree to the
following amendments to the Contract for Professional Services for Street Preventative Maintenance Program
Year 4(E162611:
Original Contract August 9, 2016 Administrative Approval $49,760.00
Amendment No. 1 September 20, 2011 Motion No. 2016-127 $676,269.00
In the Original Contract, Exhibit A, Scope of Services shall be amended as set forth in the attached Amendment No.
2, Exhibit A.
In the Original Contract,Exhibit A, Fees shall be amended based on the modified scope of services in the attached
Amendment No. 2, Exhibit A for a fee not to exceed$6,250.00,for a total restated fee not to exceed$732,279.00.
All other terms and conditions of the August 9, 2016 Contract for Professional Services between the City and
Consultant,and of any amendments t• that contract,which are not specifically addressed herein shall remain in full
force and effect.

AMENDMENT NO.2—Design Repairs to Concrete Box Culvert on N. Carancahua Street
LNV Project No. 170310
Dear Mr. Edmonds:
Please allow this to serve as our formal proposal for a contract amendment, as requested on the above
referenced project. The purpose of this amendment is to provide additional services to assist the City in
making repairs to a damaged concrete box culvert on N. Carancahua Street at Trinity Towers, to be performed
under the current SPMP FY2017 Street Overlays construction contract.
Scope of Services:
Provided by LNV
a. Conduct site visit to observe existing conditions prior to removing top on box culvert;
b. Develop scope of services and prepare contract amendment;
c. Provide one (1)person qualified in confined space entry;
d. Provide sketches/measurements/photos on existing structure;
e. Develop general notes and estimated quantities for repairs;
f. Prepare drawings for repairs(plan,section view,notes and specifications);
g. Meeting with Contractor& Inspector to discuss recommended repairs;
h. Internal LNV QA/OC review;
i. Submit draft plans to City for review;
j. Review and address City comments;
k. Submit final drawings to City;
I. Site visit during construction of the repairs.

a. Provide record drawings for existing box culvert, if available;
b. Provide personnel(attendant and supervisor)qualified in confined space entry for topside safety;
c. Provide TCP and traffic control during field investigation;
d. Provide any permitting necessary for this work;
e. Prepare construction contract change order for repair work.
Schedule:
TASK WORKING DAYS
Draft Plans 17 (following receipt of NTP from City)
Final Plans 8 (following receipt of all City comments)
Fee:
The total lump sum fee for these services is$6,250.
