("Consultant"), agree to the following amendments to the Contract for Professional Services for Cefe Valenzuela Landfill Disposal Cell Interim Cover(Project No. E110611: Original Contract January 22, 2013 _ Ordinance No. 029728 $475,465 Amendment No. 1 September 26, 2016 Administrative Approval $4,000 Amendment No. 2 December 28, 2016 Administrative Approval $5,000 Amendment No. 3 January 19, 2017 Administrative Approval $1,500 Amendment No. 4 February 3, 2017 Administrative Approval $49,500 In the Original Contract,Exhibit"A",Section I.Scope of Services shall be amended as set forth in the attached Amendment No. 5, Exhibit"A". In the Original Contract, Section III. shall be amended based on the modified scope of services in the attached Amendment No. 5, Exhibit"A"for a fee not to exceed $45,000,for a total restated fee not to exceed $580,465. All other terms and conditions of the January 22,2013 Contract for Professional Services between the City and Consultant, and of any amendments to that contract, which are not specifically addressed herein shall remain in full force and effect. Valenzuela Landfill - MSW Permit 2269 Temporary Authorization: Staging, Processing, Storage Area and Use of Water Treatment Plant Residuals as Alternative Daily Cover Dear Mr. Mikolajczyk: As requested by the City, CP&Y, Inc. (CPY), respectfully submits this proposal to provide Engineering Services in connection with the operation of the Cefe F. Valenzuela Landfill. The proposed Scope of Services includes the following. Scope of Services CP&Y will provide, on behalf of the City of Corpus Christi, and in accordance with the requirements stated in the TCEQ approval letter of May 18, 2017: • TCEQ Notifications On behalf of Corpus Christi Solid Waste Operations, CP&Y will prepare a letter to the TCEQ Region 14 office to notify of the use of the ADC materials. The letter must be submitted to the TCEQ Region 14 office at least 10 days prior to the use of the ADC materials, to allow TCEQ inspection and monitoring of the ADC performance during the trial period. • ADC Operating Plan CP&Y will prepare an operating plan describing the blending and use of the ADC. The plan will be prepared with input from Corpus Christi Solid Waste Operations staff, with consideration of equipment availability and limitations, operations details, schedule preferences, and other relevant factors. NO. 5 HIBIT "A" Page 1 of 3 Partners for a Better Quality of Life su 1) A description and minimum thickness of the ADC material to be used; 2) The effect of the ADC on vectors, fires, odors, and windblown litter and waste; 3) The application and operational methods to be utilized at the site when using the ADC; 4) Chemical analysis of the ADC materials; 5) Any other pertinent characteristic, feature, or other factors related to the use of the ADC. The initial Operating Plan will be developed prior to blending of ADC materials or deployment of ADC. The Operating Plan will also include the frequency, sample method(s), and appropriate chemical analysis of the ADC. Changes will be made as needed during the first two months of operations, and the Operating Plan will be submitted to TCEQ with the second two-month status report. • Monthly Site Inspections and Monitoring Reports (Initial 6 Months) During the initial 6 month period of the Temporary Authorization, CP&Y will make monthly site visits to observe and provide feedback Corpus Christi Solid Waste Operations as appropriate on methods and efficiencies, process modifications, any associated safety or compliance concerns, and other related operations issues. CP&Y will also prepare and submit the two-month status reports required by TCEQ during the first 6 month period. • TCEQ Extension Letter Subject to a successful initial trial period, on behalf of Corpus Christi Solid Waste Operations, CP&Y will prepare a request to TCEQ for a six month extension of the trial period. • Monthly Site Inspections and Monitoring Reports (Additional 6 Months) Following the initial 6 month period and TCEQ approval for a six month extension of the Temporary Authorization, CP&Y will make monthly site visits to observe and provide feedback Corpus Christi Solid Waste Operations as appropriate on methods and efficiencies, process modifications, any associated safety or compliance concerns, and other related operations issues. CP&Y will also prepare and submit the two-month status reports required by TCEQ during the 6 month extension. The deliverables may be utilized in preparation for modification of the operating permit for the Cefe Valenzuela Landfill if the City of Corpus Christi chooses to continue the use of WTR/Mulch Blend as an Alternative Daily Cover. For the work efforts to be performed by CP&Y, as noted above, we propose to be paid a lump sum fee of$45,000. The fee is based on the time requirements and billing rates of the CP&Y staff. The CP&Y staff expected to be used for the project are: Tewobista Metaferia, P.E. T. Halliburton "Hal" Wood, P.E. William R. Hindman, P.E. The fee for each of the items listed in the Scope of Services is provided below. TCEQ Notification $1,000 ADC Operating Plan $2,500 Monthly Site Inspections (6 months) $2,750/Inspection Two-Month Reports (3) $1,250/Report TCEQ Extension Request $1,000 Monthly Site Inspections (+ 6 months) $2,750/Inspection Two-Month Reports (3) $1,250/Report CP&Y expects to make an initial site visit as the initial ADC Operating Plan is developed. CP&Y believes monthly site visits, normally 1/2a day on-site will provide significant value to the Corpus Christi Solid Waste Operations.
